In partnership with Westchester Jewish Community Services (WJCS), the Town of Cortlandt offers a free service, Senior Care Management, for older adults in the community. This service is provided by a licensed WJCS social worker, who helps older individuals age safely in the homes and neighborhoods they love, while providing support and peace of mind to the client and their family members/caregivers. Some of the services provided include answering questions and assisting with benefits and entitlements, referrals and connections to local, state, and federal resources/programs/services, in-home assessments for care plans, guidance on senior living options, and ongoing case management as needed.
